Yes, you will need to complete updated intake forms and consent documentation to ensure we have your most current information and to comply with our clinical procedures.
If you are in Illinois, North Carolina, or South Carolina, while we have your medical records, you will need to fill out new intake forms.
If you are in Texas, we will not have your medical records. You will need to complete our new patient onboarding process including intake forms and a good faith medical exam prior to your first treatment.
